Please choose from the following options to book your appointment.

Devine Nutrition Inc. is in network with BlueCross BlueShield, UnitedHealthcare, Cigna, and Aetna. If you have coverage with one of these providers, we will check your benefits for you to see what is covered once you are scheduled. If you are not covered by one of these plans, we are happy to discuss our self pay rates. In our first visit we review your current diet, any labs, and your goals and then make a plan for you. Follow-ups will focus on goal setting and further education along with food log review.

All forms and insurance information needs to be complete within 3 days of your scheduled appointment.

At this time, Devine Nutrition is doing both in person visits at the Mokena Park District or Palos Hills offices or virtual sessions.
